What you'll do
- Provide home-based physical therapy to patients who have lost mobility, strength or function due to illness or injury
- Work as part of a home health team that may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals
- Observe movement, complete assessments, document progress and adjust the care plan as needed
- Guide exercises and hands-on therapy to manage pain and increase mobility
- Educate patients on their plan of care, goals, exercises and self-care
- Coach family on the plan of care, therapy goals and safely assisting with exercises
What you need
- Graduate of an accredited Physical Therapist program
- Active Physical Therapist license in Michigan
- Home health experience preferred; new grads welcome
- Current CPR certification
- Reliable transportation, a valid driver's license and current auto insurance
