As a Nurse Practitioner with Interim HealthCare of West Michigan, you'll bring advanced practice skills to patients at home, working closely with physicians and the care team.

What you'll do
- See patients in their homes and complete advanced assessments
- Work with physicians and the interdisciplinary team on the plan of care
- Document visits and communicate changes to the care team
- Educate patients and families on the plan of care
What you need
- Active Michigan Registered Nurse license with Nurse Practitioner specialty certification
- National NP board certification
- Home health, hospice or palliative care experience preferred
- Reliable transportation, a valid driver's license and current auto insurance
