As a Medical Social Worker with Interim HealthCare of West Michigan, you'll support patients and families facing serious illness, connecting them with resources and helping them through difficult decisions as part of the home health and hospice care team.
Schedule
Daytime home visits with flexible assignments.
What you'll do
- Complete psychosocial assessments of patients and families in the home
- Connect patients and families with community resources, financial help and support services
- Provide counseling and emotional support, including through end of life and bereavement
- Help with advance care planning and care decisions
- Take part in interdisciplinary team meetings and document visits and plans
What you need
- Master's degree in Social Work (MSW) from an accredited program
- Michigan social work license
- Home health, hospice or medical social work experience preferred
- Reliable transportation, a valid driver's license and current auto insurance
